A joint injury from a car accident is a sprain, capsule tear, or cartilage injury to any joint loaded by the crash: the facet joints of the spine, the sacroiliac joints of the pelvis, or the wrist, elbow, shoulder, hip, knee, and ankle. When a crash moves a joint past its normal range, the ligaments and capsule tear. The joint then swells, stiffens, and stops moving correctly.

Joint symptoms usually take days to become obvious, because the swelling inside the joint builds gradually. A wrist that "felt jammed" at the scene cannot bear weight by day four. Florida's 14-day PIP rule requires initial treatment within 14 days of the crash, so the exam has to be scheduled on the crash date, not the stiffness date.

How joint injuries happen in a collision

Joints are injured when a crash forces them beyond their normal range. The facet joints of the neck and back are compressed and sheared as the spine whips. The sacroiliac joints, where the spine meets the pelvis, are loaded when the lap belt holds the pelvis while the trunk keeps moving. Those two are the most common joint injuries after a crash, and the ones most often missed.

The wrists and elbows are injured by bracing on the steering wheel. The shoulder is loaded by the seat belt. The knee strikes the dashboard. The ankle twists under the pedal. How we evaluate a joint injury

Each involved joint is examined for swelling, tenderness, and warmth; its range of motion is measured and compared to the other side; and its ligaments are stress-tested one at a time for laxity. X-rays are ordered when there is bone tenderness, when the joint cannot bear weight, or when instability is found on testing. How we treat joint injuries

Chiropractic adjustments are the core treatment for a joint that has stiffened or locked after a sprain: they restore normal motion so the ligaments heal at the right length. Class IV laser therapy on the LightForce system reduces swelling in and around the joint in the acute phase. Therapeutic ultrasound is used on the joint capsule, and electrical stimulation reduces spasm in the muscles crossing the joint.

Recovery and what gets documented

A mild joint sprain improves over a few weeks. Ligaments heal slowly, so a moderate sprain takes longer, and an unstable joint that does not tighten up with time goes to an orthopedic specialist. Joints treated early regain full motion more often than joints left to stiffen. We re-measure range of motion and re-test stability at set intervals.

When the emergency room or a specialist comes first

Go to the emergency room first if a joint is visibly deformed or out of place, if you cannot bear weight or use the limb, if the joint swelled up within an hour of the crash, or if the limb below the joint is numb, pale, or cold. What is a facet joint injury?

Facet joints are the small paired joints at the back of each spinal level that guide its motion. In a crash they are compressed and sheared as the spine whips, and the capsule around them sprains. They are the most common source of lasting neck and back pain after a collision. Specific orthopedic tests reproduce the pain, and adjustments and laser restore the joint's motion.

Why does one side of my low back hurt after the crash?

One-sided pain low in the back, near the dimples above the buttocks, is usually the sacroiliac joint, where the spine meets the pelvis. Is it safe to adjust a joint that was just injured?

Adjustments are safe once the exam has ruled out fracture, dislocation, and instability, with X-rays when indicated. In the acute phase Dr. McNamara-Krauss uses laser, ultrasound, and gentle mobilization rather than a full adjustment, and progresses as the joint tolerates it. A joint that tests unstable is not adjusted until imaging clears it.

When is a joint injury beyond what this office treats?

A dislocation, a fracture into the joint, or a complete ligament tear that leaves the joint unstable needs the emergency room or an orthopedic surgeon first, not conservative care.
