A knee or leg injury from a car accident is damage to the knee, hip, ankle, or the muscles and tendons between them, caused by impact with the dashboard, the pedals, or the door, or by the leg being braced at the moment of the crash. Common patterns are a bruised or sprained knee from the dashboard, a strained hip flexor from bracing on the brake, and a sprained ankle from the foot twisting against the pedal.
Leg injuries usually hurt within hours, and they are the injuries most likely to be dismissed as "just a bruise" while the person focuses on their neck. Florida's 14-day PIP rule applies to the knee the same as to the spine.
How knee and leg injuries happen in a collision
In a frontal crash the driver's knees travel forward into the lower dashboard. That impact bruises the kneecap and the fat pad behind it, and if the force is high it drives the shin bone backward and sprains the posterior cruciate ligament. This is called dashboard knee. Side impacts push the door into the outside of the knee and sprain the MCL on the inside.
Bracing is the other mechanism. Drivers who see the crash coming stand on the brake, which loads the hip flexor, the quadriceps tendon, and the ankle at the instant of impact. Rear-end hits on Federal Highway and the Sawgrass Expressway are often braced hits. The foot twists under the pedal and sprains the ankle.
How we evaluate a knee or leg injury

Knee ligaments are tested one at a time for laxity. The kneecap is checked for tracking and tenderness, the hip for impingement, and the ankle for ligament injury. The low back is examined as well, because leg pain often starts in the spine.
X-rays are ordered when you cannot bear weight, when there is bone tenderness, or when the knee swelled within an hour of the crash, which suggests bleeding inside the joint. An MRI is ordered for suspected ligament or meniscus tears, through our imaging and specialist referral process.
How we treat knee and leg injuries
The knee and leg are extremities, so the full set of tools is available. Class IV laser therapy on the LightForce system reduces swelling in a bruised knee or sprained ankle in the acute phase. Shockwave therapy on the Chattanooga system treats tendon injuries at the kneecap, hamstring, and Achilles once the acute phase has passed. Laser and shockwave are never applied to the same area.
Therapeutic ultrasound is used on ligament sprains and the hip flexor. Electrical stimulation reduces spasm in the quadriceps and calf. Chiropractic adjustments restore motion to the hip, the sacroiliac joint, and the ankle when they have locked down. When leg pain traces to a lumbar disc, spinal decompression on the Antalgic-Trac system addresses the source.

A bruised knee or a mild ligament sprain improves over a few weeks. Tendon injuries and higher-grade sprains take longer, and a torn meniscus or ligament that is not healing goes to an orthopedic specialist. When the emergency room or a specialist comes first
Go to the emergency room first if you cannot put weight on the leg, if the knee or ankle is deformed, if the knee swelled up within an hour of the crash, if there is an open wound over a joint, or if the calf is swollen, warm, and painful, which is a sign of a blood clot. What is dashboard knee?
Dashboard knee is the injury caused when the bent knee strikes the lower dashboard in a frontal crash. At low speed it bruises the kneecap and the fat pad behind it. At higher speed it drives the shin backward and sprains the posterior cruciate ligament. A knee that feels unstable going down stairs suggests the second, and gets an MRI referral.
My knee swelled up after the crash. Is that serious?
Swelling that appears within an hour of the crash usually means bleeding inside the joint, which points to a ligament tear, a fracture, or a cartilage injury and needs X-rays and often an MRI. Swelling that builds slowly over a day is more often a bruise or a mild sprain. Fast swelling is referred for imaging before treatment starts.
Can shockwave therapy be used on the knee?
Shockwave on the Chattanooga system is used on the knee and leg because they are extremities, not the spine. It is applied to tendon injuries at the kneecap, hamstring, and Achilles once the acute inflammation has settled. It is never combined with laser on the same area; Dr. McNamara-Krauss chooses one for each structure and records the choice.
Why does my leg hurt when the crash injured my back?
Leg pain that runs from the buttock down the back of the thigh, with tingling or numbness, is usually a lumbar nerve root being irritated by a disc, not a leg injury. The exam includes the low back for that reason. If the source is the spine, decompression and laser on the spine are the treatment, and the leg improves as the nerve is freed.
How soon after a crash should I have my knee examined?
Within the first few days, and in any case within 14 days, because Florida PIP requires initial treatment within 14 days of the accident for the medical benefit to apply.
